How about a ticket to something he likes, like a car race, a concert that is coming to town, a baseball or basketball or hockey game? Or if you have a smaller budget, how about a subscription to Sports Illustrated?

Get a toilet seat (new or used) .
On the outside(top) of the lid, with marker or paint write;
“40 years old?”
Then the inside, lift the lid and use the seat part as a frame for a picture of the birthday boy and around the picture on the seat or on the back (inside) of the lid write;
“no wonder you look pooped!”
