It should be an objective analysis of the task and
What obstacles were encountered .
How the challenges were handled &
Ask could there have been a better way .

Were deadlines met ?
Were delays due to foreseeable issues like staffing ?
Or did a hurricane flood out the computer storage room ? (act of god , somethings no one can prevent)

It should be objective and not a finger-pointing / blame venting session .
