Where can I get the forms and is there a website I can go through that is credible? How much on average is it?

Most states allow you to register online. Go to your state's official website and click the BUSINESS link. It will take you maybe 30-45 minutes and cost about $100 (depending upon the state).

Go to Secretary of State website of your State or go to Department of Corporation. You can download application forms and see a list of fees. Otherwise, ask your accountant.

Starting a corporation as an S-corp, or a C-corp have advantages and disadvantages. I HIGHLY advise talking to a legal person before doing anything. Launching any kind of corporation will cost you over $1k, so discussing it with an expert may add a couple hundred bucks, but it's really well worth it.
