When people say “LOL” do you really think half the people are really Laughing Out Loud???

No. It's just a way of saying that they think it's funny. But sometimes, it can be mere sarcasm.
There's another one popping up: ROFL- Rolling On Floor Laughing.

I may be laughing my socks off on the inside when in the office or in public... or a full blown laughter when alone or with friends!

In what ever the case... I hate people who fake LOL's... if they fake that, they could fake all else.

When I say lol I usually just chuckle or smile, but if I really 'LOL' I put ROFL or LMAO

no most of the time i say that because i have nothing better to say and i dont want the person to think that they were funny but i had no response
