Please help. I am in the process of petitioning for a legal name change and I have to type an order for the change so the judge can sign it. There is no form in kansas for it and I have to do it myself. The judge says it looks like a journal entry/ court order. can someone plase help me??? 10 points best, quickest answer!!! Thanks!

Hey, Buff....Here's the form for an Order Changing the Name of an Adult. Take what key phrases apply to you from the form and delete the ones you don't need. This one is for Texas but one for Kansas is probably fundamentally the same...

Go to your nearest law library and consult a publication called "AmJur Legal Forms". This is a multi-volume set which has forms for most purposes.

The law librarian should be able to assist you in finding the correct volume for the order.

It is also possible that these are available at the law library electronically via WestLaw, Hein Online, or a similar service.

Go to the court clerk's office. Look for another recent name change case -- there should be some sort of index by case type -- ask the clerk. Pull the file & copy the order.

Check with local Office Supply stores. They carry many blank, legal forms.
