You can alter the path little bit but this is the only choice:
UK-France-Germany-Austria-
Slovenia-Croatia-Serbia-Bulgaria-
Turkey (upper west side; Edirne) -
Turkey (via Istanbul, to south coast; Mersin)
From Mersin there are ferrys everyday to Cyprus. They are so cheap and take about 3-4 hours...
My aunt used to visit us from UK, she couldn't fly either. So she always drove to Cyprus. That was her route. So I remember;
Whole trip should take 3-4 days. Depends how much you drive in one day! If you think you can drive more than 12 hours a day, you ll get there less than 3 days...
But its a very long way to drive so I ll recommend at least 2 drivers and very comfortable seats!...

He will have to consider being in a plane for about 5 hours, (from boarding to getting out at the other end,) or being in a train, car or on a ferry for 5 days or more.
He might prefer 5 hours of misery above 5 days of being uncomfortable, or being miserable because not being able to leave at his own wish.
And flight crew is trained in care for people who suffer from phobia's.
If he is comfortable in his own car, driving it himself, suggest him to take it slow, doing 6 hours of driving a day, stopping often for sightseeing and meal breaks. It will take more than 5 days that way, but it will be an enjoyable travel.
He will still have to take a boat to Cyprus, that is un-avoidable.
Maybe a cruise from Italy or Spain is a way to avoid a long drive through former Yugoslavia and Turkey.

Drive to either Venice or Brindisi, take a ferry to Igoumenitsa, drive to Piraeus and then ferry to Lemessos in Cyprus, or drive straight to Piraeus and then take the ferry. UK to Venice is a long days' drive,(two drivers), Brindisi or Piraeus direct will mean a night stop

he'd have to drive, then get a ferry to limassol or larnaca.
ive got some family who are planning to move out here so ive been checking the web for info on their best route, as they want to drive over. ive found a good forum that people who have travelled by car tell you where to go and how long it'll take, so read this and im sure you'll find the information on how he'd get to cyprus from england by car and ferry

How claustrophobic is he?
I suffer from claustrophobia but British Airways now has a service class between economy and business (called World Traveller Plus or Club Europe). It's nowhere near as expensive as business class but feels much, much better than economy on a claustrophobia front. That may be worth trying?

He really should fly. Train,boat or car the journey will be far worse for him. Nobody much likes flying, it is mind over matter to get to places. You can buy rescue remedy from the chemist. It is quite good.

ABSOLUTELY TERRIBLE: No entertainment -Unfriendly staff Unorganized boarding, they DO NOT allocate seats And there speedy boarding is a joke!
