It was the U.S.S. Parle (DE-708) , named after Ensign John Joseph Parle of Omaha, Nebraska. Died July 17, 1943 from wounds received while aboard L.S.T. 375 and posthumously awarded the Congressional Medal of Honor.

I believe that you're thinking of the USS Parle (DE-708).

As for it being the last active DE--maybe. The biggest problem is that some DE-class ships were brought into service after the was, some converted to other roles, and finally, the Oliver Hazard Perry-class frigates were originally designated as 'DE' before someone decided to go with 'frigate' instead.
