Most of the time the blood is real, but they cut themselves in specific area, such as the high forehead near the hairline... That is because it bleeds profusely, making it look worse than it actually is. Also, it doesnt hurt nearly as much getting hit there, it heals quickly with almost no visible scar.

I saw a documentary years ago where a former wrestler showed a small chip of razor blade with tape on it. They would hide this until the right moment, then take it out and slice their forehead with the small razor chip, then hide the chip back in a wristband or toss it somewhere.

Razor blade. It's amazing how a little cut in the forehead can bleed like crazy.

There was a 5 part news article years ago that showed a picture of the small razor wrapped in tape they use to cut themselves.

i haven't paid too much attention to wrestling lately, but in my
day we used the tip of a razor blade....did you ever notice
wrestlers with a little tape on their fingers? This was a common place to hide a tip...all you had to do was a couple of
little gigs, your heart raate was elevated....hence the good blood flow from a small cut....never bladed myself, though my
brother did from time to time....
