Chances are, the court did not issue a warrant or take any other action against you. However, because The United States Constitution guarantees the right to trial by jury, failure to attend as directed may subject you to being held in contempt of the Jury Service and Selection Act. Contempt penalties range from a $100 fine to three days imprisonment, or both.
That is, IF the court bothers to persue it. You should contact the Court as soon as possible, explain the situation, find out if any action is being brought against you, and if so, find out what you can do to fix the situation. Hopefully, the court didn't take any action and you're in the clear. Good Luck!
(P.S. In most states, claiming that you did not receive the letter due to the state having the wrong address, will only get you in more trouble. This is due to most (if not all) states having laws that require you to update your driver license file within two weeks of a change of address. Failure to do so may result in a fine. Contacting people for jury duty is one of the many reasons for this law.)

You need to check and see if you have a warrant. Some judges issue warrants while others let it go. Call the number on the paper, explain the situation, and see if you can get another date.

Most states accept one miss ... but you need to call in and tell them what happened. They may potentially reschedule you.
There may be a warrant for your arrest for contempt of court (i.e. not appearing). However, if you call in ... well, people screw up and you aren't the first.
However, if you keep doing it, expect to spend some time behind bars.
