What do I do w/ the stock issuances, net income, cash dividends to find the equity?

Please be more specific. Just based on your question I would say that you should subtract cash dividends from the stock issuance and net income. However, there is probably other equity on your balance sheet too (such as retained earnings).

They don't give you a balance sheet? Retained earnings is previous years' net income. If it is a new corporation, that is the only time there wouldn't be retained earnings.
