Yes all the time. I do not allow shoes in my house and I am pretty strict about that rule. When I go to someone elses house I feel I should behave the same way I expect guests to behave at my home. So when I get to the door I quickly look for the clues of a shoes off household (i.e. shoes at the door, a sign, or whoever opens the door is wearing shoes or not). If I see a bunch of shoes at the door I just slip mine off without saying anything. If it is not that obvious I will just say "where do you want me to leave my shoes." If they say it not to worry about it I just tell them "it is a rule in my house that everyone that enters take their shoes off." This way if they ever come to my house they will know how I am.

Yes, I feel it's just common etiquette. Just as I like people to take off their shoes at my house.

I always take my shoes off in other people's house because i'm used to it from my house 'cos i've got a cream carpet!!

Yes it's kind of respect and it's not my house. If it's my house, i wear the shoe right in to my room because my brother maid doing the job and he is paying for it.
