Forming an LLC in the USA should take no more than 1 week once the documents are sent to the Secretary of State. You can do it on line for $200-$500. I would urge you to speak with a CPA, EA, or Attorney before you do anything. I provide a link below to 2 companies that file the paperwork for you.

I use company formation agents who are not cheap and they normally manage to complete all the paperwork within 24 hours. So the company could be formed by the next working day at the latest.
I would suggest you search for company formation agents online and read up on the details. The cheap ones should still be able to complete everything in a few days, although you may have more costs to incur later on.

go to www.companieshouse.gov.uk
this is where all info relating to setting up a company and a list of all registered companies are held.
you will find all the info you need right here and they also provide a service for setting a company up for a fee.
you could fine a company setup firm that could do it cheaper if you just google it, but ultimately, all companies that are set up go through companies house and all the details are held on the website.

Do you mean Limited Liability Company? You can register online through your state's official website (click on the BUSINESS link). Will cost you about $100 (depends on the state), and take less than on week.

you have to think of a name, who is incharge etc. Also you need to determine the nature of the company. The you need to fill in a memorandum of association and articles of association and send them to the company house who issue you with status and registration.
Not sure on how long it would take - probably about 2-3 months
