The windows start up sound can be changed by going to Control Panel. Goto Sounds and Audio Devices Properties. Click the tab Sounds. Down where it says Program events, scroll down to start windows and click it. The browse button at the bottom right click it and pick a new sound. You can record a file and use it for your own start up for MAME, I made one of "Arcade Classics Activated"

To have MAME start when windows starts. Create a Short cut to the MAME gui (startup) and place it in the START up folder of windows.

You are using MAME32 now right?

The easiest way Start MAME, start a game, once it has loaded press the TAB button, a box will open and choose Input (general).
Now in this area you will be mapping the controls. So you select each action and hit enter then press the button or controller action you want it to correspond with.

But really you do need to read as much as possable about MAME and related Items to enjoy it to the fullest. Cheers..
