Some people in ill health do die on the toilet because of the stress it takes to have a bowel movement. And most people lose control of bowel and bladder when they die, and stuff does come out. I have had to clean it up for the sake of family and everyone else.

While the body will probably excrete its waste materials due to the relaxing muscles and pressure, this does not necessarily happen right away. It may in fact NOT happen if there is very little in there, for instance if they had just used the restroom.
In a traumatic death like Hanging, though , the Stuff comes out then and there i have heard.

I've heard it could happen in the morgue. That's why they have to close up everything.
