MOS burgers is definitely the place to go! Oh, how I miss the burger with jalapenos ;)
Yoshinoya is also great. Only 280 yen (when I lived there in 2003/04) for a bowl of tasty gyudon.
The most popular fast food shops would be MOS, McDonalds and Yoshinoya. Wendy's are good too but there aren't that many around in comparison.

There are many.
Some are chains, some are just private little stalls.
I'll go over a few of my faves:
- any sobaya (noodle stall). You can sometimes get a nice hot bowl of soba or udon for as little as 300 yen.
- most ramenya are good (ramen noodles as opposed to soba or udon). A lot of people love these noodles. They tend to be a bit more flavourful than the others. Get a side dish of gyoza, and perhaps a beer, and that can really hit the spot.
- for burgers, there's MOS, who make some delicious stuff. It's not really fast though. Be prepared to wait a little longer than say Lotteria.
- Lotteria is good for burgers, cheap, and often has a couple types of burgers not found at McD's.
- convenience stores can often provide enough for a lunch. Try some delicious onigiri (rice triangles), and supplement it with a drink, some sweets, etc. Family Mart has pretty decent fries too.
- check out any food stall you might find. Sometimes they're in touristy areas or at festivals. You can try nice things like grilled squid, a baked potato with butter, takoyaki, okonomiyaki, oden, etc.
- drop into a supermarket and grab bread, cheese, and luncheon meat, and make a fast sandwich.
- bento! You can get bentos in most supermarkets and convenience stores, and there are also shops that specialize just in making cheap bentos for take-out! Bento Man is one big chain.

There are also many Japanese fast food chains. Beef bowl, Udon noodle, Soba noodle, Sushi, etc.
Onigiri(rice ball) is famous fast food. Some people say Bento and Japanese curry rice are also fast food.
By the way, I used to look for the hamburger which matches for the image of the one Wimpy eat in the cartoon "Popeye the sailor".

The meat of the hamburger is thick and it looks plain burger. I guessed he added some extra mustard and ketchup, cos he doesn't look young. I couldn't find the one at McDonald's, WENDY'S, and B.K.
One day I noticed MOS sell simple hamburger (not MOS burger in their menu). So I ordered a hamburger but I said "No onion, but double mustard and double ketchup please."
You know what. The taste exactly matched for the image of Wimpy's hamburger.

Some of my favorites are:
MOS Burger. Love the sauce on their cheeseburger!
PEPPER LUNCH. I am addicted to their steak. They give you raw meat on a hot plate, and you cook it on your own, adding sauce depending on your preference.
YOSHINOYA. I love their Yakiniku version eaten with raw egg. Yum!yum!
They also have WENDY'S HAMBURGERS in Japan.

if you go to the shopping malls and go to the food courts, they are packed with delicious fast foods, lots of variety, and cheap.
also MISDO (mr.Donuts) delicious
Mcdonalds- eat their teriyaki burger. its soo good. 280yen.

Stay away from MOS burger! Its for pxxsies. I can down that crappy thing, they call 'burger', in 2 bites. My kinda burgers are Big Mac, Whopper, and best of all Bob's Giant Burger. Unfortunately, Bob's not a chain store and you can only get it California, along Fremont Blvd.
