From IMDB.com: The character of Rose is partially based on California artist Beatrice Wood, who died in 1998 at the age of 105.
When James Cameron was writing the movie, he intended for the main characters Rose DeWitt Bukater and Jack Dawson to be entirely fictitious. It was only after the script was finished that he discovered that there had been a real "J. Dawson" who died aboard the Titanic. This "J. Dawson" was trimmer Joseph Dawson, who had been born September 1888 in Dublin, Ireland. His body was salvaged and buried at Fairview Lawn cemetery in Nova Scotia with many other Titanic victims. Today, his grave stone (#227) is the most widely visited in the cemetery.

No, the chracter of Rose is based on a real life artist named Beatrice Wood. However, "Rose" can also be seen as a composite of many of the society girls of the 20 years before and after the year 1900 that were often forced by their families to marry for money or status rather than love. People like Consuelo Vanderbilt... as well as a symbol of the way the lives of women in general changed over the century, too.

well i cant say 100% for sure,
but there is a possibilty.
I once heard soon after it came out, that she is based on a woman (not sure of name) in the 80's who had the visit to the titanic and the Kelvish with Robert (cant think of his last name).
As for Jack and that story dont know.

Definitely fictional, actually, there is nothing remotely historically accurate about that movie.
