The answer would be no to both. Here is an example of on base percentage.
Divide the total number of hits, bases on balls and times hit by pitch by the total of at-bats, bases on balls, times hit by pitch and sacrifice flies. For example, if Shawn Green has 619 at-bats, 184 hits, 72 bases on balls, 5 times hit by pitch and 5 sacrifice flies, his on-base percentage is .372 ((184+72+5)/(619+72+5+5)).

No it does not count for the OBP. The foursituations that don't cpntribute to OBP is Fielder's Choice, Fielding Error, Fielder's Obstruction or Catcher Interference.

Yes, it counts, but doesn't improve. Reaching by error or FC is scored as going 0-1. The batter's efforts put him at standard risk of being put out, but he was not due to a mistake or decision by the defense.
