(sqrtx^2+4)(-sqrtx^2-4)
= -(sqrtx^2+4)(sqrtx^2+4)
= -(sqrtx^2+4)^2
= -[ sqrtx^2(sqrtx^2+4) + 4(sqrtx^2+4) ]
= -[ x^2+4sqrtx^2 + 4sqrtx^2+16 ]
= -[ x^2 + 8sqrtx^2 +16 ]
= -x^2 - 8sqrtx^2 -16

Is it (sqrt(x^2)+4) x (sqrt(x^2)-4)

sqrt(x^2) = x

use the formula a^2 - b^2 = (a+b)(a-b)

then the expression becomes (x+4)(x-4) = x^2 -16

I hope I got the original expression rightly.
