Serious answers about physical or mental torture techniques... example: "being married to my wife" or "having to listen to (sucky band)" are NOT what I'm looking for!

This morning, I listened to a podcast on Vlad the Impaler. Apparently, he liked to torture his victims by having his henchmen cut them open alive and impale them on huge poles sticking out of the ground.

There are lots of medieval torture devices around. Heck, even the rock band Iron Maiden was named after, what else, the "iron maiden" killing thingy (a coffin with spikes inside). You might want to google "medieval torture devices" and you'll get a huge laundry list of horrible instruments of terror. I'll include a website I found while doing a google search.

Torture and killing are, unfortunately, a part of human history; everything from impalement to gas chambers. It's incredibly sad.

The Viet-Cong used to do this to captured P.O.W.'s, they would capture a rat and not feed it for weeks on end, then tie up the soldier to where he cannot move or squirm, then cut a slit in their stomach, and place the rat on the wound. The rat would hear the soldiers beating heart and chew his way to the "threatening sound" eventually eating a hole in the heart.

Pretty gross, but true.

Or, believe it or not, Chinese water torture is also quite maddening, because think about a single drop of water striking you on the head over and again for weeks on end. You would not be able to sleep, so truly dreadful if you ask me.

Or be forced to slide down razor blades into a vat of rubbing alcohol.

Historically speaking, the romans considered dying on a cross to be the worst form of death. Not only do huge nails go through your hands, but its a slow and painful death by suffocation. You are basically hanging by your arms, cutting off oxygen flow to your abdomen. Being whipped with glass and nails and anything else sharp before hand doesn't help. Yes, this is what Jesus went through for you so you could spend eternity in Heaven with Jesus.
This is sounding really cheesy right about now, but its the truth.

How exactly are you going to measure that? Is it worse to be burned alive, or flayed slowly, or crucified, etc.?

I'd say fire evokes a powerful fear in most people... so some kind of burning.
