A miscarriage can occur even the very first day of pregnancy, of course, a woman may not even know she is pregnant then. Any time what is deemed 'products of conception' are expelled, it is considered a miscarriage.

I believe a miscarriage can occur at any time during a pregnancy, but in the latter stages it would be considered a premature birth. Miscarriages are generally more common during the first trimester.
