The ticket is/was your responsibility. The credit card thing and the address change are against the law. You need to press charges and get an attorney for prosecution.

Everyone is correct in telling you that you need an attorney to clear your name. You also need to contact the three credit reporting agencies yourself by mail stating that your identity has been compromised and detailing the 5 credit cards you mentioned so that they can be taken off of your record.

The agencies are: TRW, Experian and Equixfax. He threatened your life with that final message. The fact that you are writing here shows that you either don't take him seriously or are reticent to press charges for whatever reasons. Common sense tells you to take care of this immediately.

Finally, deal with your psychology of allowing your personal responsibilities to be paid by someone else. Do you expect others to take care of you? If you are over 30 years old you should know better. If you are in your 20's it is still understandable. Be careful who you date or allow into your life and always handle your own obligations and financial responsibilities. You are responsible for you.

see a lawyer. hire him with your money so you know they represent you properly.
lawyers (even brothers in law) are all pretty slippery.
