while mentioning the roots of a number, we write as + or _ . My question is, should we mention + or _ symbol in the previous step also along with the square root symbol.

in fact no
when i say what is the square root of 9 i mean 3 or -3
but when i write root 9 i mean positive root 3
-root9 i mean the negative root -3
so we say +/- in two cases
1) when you are asked what is the square root of num
2) when given x^2 = 4 then x should equal +/- 4
but with written given root we deal with it as it is with the given sign besides it if there is no sign so it is positive if there is - then it means the negative square root only
if both written it means get positive or negative square root

It's not necessary, it's simply to say that the square root of a numer can be + or - because -² = +

(-5 x -5 = 25 (not -25)).

You use it only before the square root symbol

we should not use + or - behind the sq.root symbol

sqrt(x^2)
=|x|

or

sqrt(36)
=6


or

sqrt((-6)^2)
=6

I dont quite understand your question but i hope u get your answer from these steps.

x^2 = 5
x = +or - (sqrt) 5
x = (sqrt)5 or -(sqrt)5
