no middleman to damage or drive up the cost of the goods... also no distributor to check the quality of the goods before deciding whether to sell them or not. i see the dilemma.
i have an impression of factory-direct products (why do mattresses always come to mind?) as being of less than the best quality... sort of like a not ready for prime time player. even if the product is manufactured to the same standards as another more popular product (and of course they'll tell you so) there's still a hint of doubt in my mind.
having not bought anything direct from the factory, i have no firsthand experience to share, only my impressions. but those impressions will likely keep me from making that first purchase in the first place!

Here, in Brazil, it can mean both.
The city where I live now has a lot of small factories of clothes, and it's much cheaper to buy directly from them than at stores. You just have to know the reputation of the place. There are many good ones, where you can buy good quality clothes for a decent price. The same goes for many other products, all over the country.
I'd say that "factory direct" means great value, in most cases. Or that's the impression I get, based on my very limited experience. :)
Edit: After reading Patz's answer, I'm not sure if we're talking about the same thing, maybe I misunderstood the meaning of this expression, in the US. Here, any factory, including big ones, with a good name and reputation, can and do sell their products "direct", frequently.

Factory direct mans there's no retail mark up. It's usually cheaper.

It seems like a gimmick, doesn't it? I would think the place you are buying from is a distributor for the factory.
