Our mind can entertain only one thought at a time,the time may be in milliseconds and may process hundreds of thoughts in succession to appear as a n extended single thought.This is the key point.Have a single thought and keep on it excluding all other thoughts.This thoughts may be verbal or a visual like a flame of a candle.When one can successfully do it for few minutes at a stretch then comes the next stage of having no thoughts or blanking off the mind.That is a difficult stage and would need a master to guide you.With mastery over a single thought,you shall have many changes coming in to life and you shall feel the difference yourself.

You don't necessarily need total peace and quiet (and sometimes, that may just distract you even more because it's jsut too quiet). As long as you truly want to get inside your own head, and be peaceful and just meditate, you can. For different people, there's different things that help them meditate. You just need to experiment. For some people, quiet, peaceful music helps. For some people, a loud room full of noisy people helps (believe it or not).

It is a process which consists of several steps. It takes time. Breathing techniques and focussing on it is one such method. Or focus on one object or entity. It is natural to get distracted from time to time but soon enough the concentration span will increase.
