Perth Australia has a population of 1,507,900 - The nearest city to Perth with a population over 1 million is Adelaide in South Australia, which is 2,104 kilometres (1,307 mi) away, making Perth the most isolated city(of over 1 million people) in the world.

The most remote, major inhabited city is probably Honolulu, but it's population is not above 1 million.

Another way of thinking--West Berlin may have been the most isolated city when the wall was up.

Well, if it's a Kazakh city, I'm gonna guesstimate that is Alma-Ata.
