Can you make your own form or does it have to be made by an Attorney? (For Banking/Mortgage Purpuses)?

Technically, a Power of Attorney, as long as it conforms to your state's requirements for a Power of Attorney, does not need to be drafted by an attorney.

However, to ensure that what you have drafted conforms to your state's requirements, and to make sure that there isn't a flaw which will create an even bigger mess later on, (that would cost you more money to undo), you should probably have an attorney draft it for you. It shouldn't be that expensive, and you could potentially save yourself a lot of money by having it done right the first time.

For a referral to an attorney, contact your local or state bar association.

They sell the forms at office supply stores. When it comes to money matters though, I would have an attorney do the form to protect you. You are basically giving your money to someone else.

It's best to go with one already drawn up. If you go to your bank, they may have a form you can fill out and have notarized rather than going to a lawyer or such and it'll not cost you anything other than maybe a few bucks for the notary if the bank doesn't offer to do it for you.
