Besides the Arab countries mentioned in earlier answers there are:
Thailand, Japan, Spain, Denmark, Nepal, Monaco.

Although Malaysia is a Republic, they have a King who is elected each couple of years from the Sultans within Malaysia.

All the European nations have Royal families which are all related to each other, sometimes five or six times over, or even more.
