I use Photoworks ALL the time and I love them (they are the first link). They'll often have specials (buy one get one free) and such, but you'll have to create an account to get the email notices about when their specials are. I HIGHLY recommend them just for their quality and this Saturday they are having a 20% off sale on their calendars.
YorkPhoto is the second and they are okay. They are having a special right now...so I added them so you could check them out.
Shutterfly also has custom calendars that you can create and I've heard good things, but I've never used them.
Calyptic looks like they make a lot of styles and also adjust prices depending on how many you want to order. Since you are looking for ordering 5, I'd check them out to see if this site may offer you the best discount.

TJ gave out some great websites, but I dont think Wal*Mart's price of $16 is bad at all. I usually see $25 and ^ for photo calendars.
