It's an observation of how well something works. Like if you have a knee injury, it would gauge how well your knee works compared to how well a healthy knee would work. In students with developmental disorders or behavioral problems, they use it to decide what interventions might be appropriate to get more appropriate behavior out of that kid.

A functional assessment is not really a test. A functional assessment is usually done by watching a person and noting their behavior. It is also done by interviewing people who know the individual well, to figure out how well the person can do with different skills. Usually the individual is not asked to do anything, but is observed going about daily tasks.
