I can really address only the death of North American Native Americans. There were other situations that were quite different and much more one-sided.
Truly, the only honest answer is "everyone". Not equally, but still, there are very few innocents involved, Native Americans included. People didn't always act from bad intentions. Many bad things happened because of bad communication, lack of willingness to change, and lack of knowledge (that's where disease fits in). Native tribes warred long before Europeans came, and they continued to do so, often using alliances with Europeans to help them. Europeans called the "Mohawks" by that name (which was Algonquian and meant "eaters of men") because the Algonquians were the Europeans' friends and the Mohawks (whose real name was something altogether different) were the enemies of both Algonquians and Europeans. The name "Apache" is Zuni for "enemy". "Sioux" was also another tribe's (I can't recall which without looking it up) word for "enemy".
A lot of the worst things happened because elected politicians did exactly what their constituents who had voted for them demanded (gee, could it be that we have not learned a thing in that area???). People wanted land, they were given land, Native Americans attacked them because they thought the land should be theirs, and the people demanded wiping out the natives.
Some native tribes got a far worse and more unfair deal than others. Research the story of the Cherokee -- it can break your heart.
I don't justify any of the bad things, but I do hate politically correct oversimplifications that ignore history.
I'm an immigrant, so I have no dog in this fight. None of my ancestors were on either side.

There was no specific person most responsible, I would put the blame on smallpox and the measles brought over by the Europeans with the Indians having no immunity to the diseases at all.

As you may recall, European settlers crossed here to get new jobs. As many migrated to the New World, they brought measles, small pox, typhoid, malaria, and etc.... The Europeans have the fault of bringing the diseases because they defeated the Spanish Armada. This is the best answer you may get. Native Americans used to live for a span of 120 years. Now, the average is 80-99.

My opinion is definitely the Spanish and the English, who both enslaved them, but I think that for destroying whole tribes it would have to be the Spanish.
The French killed when they needed to I am sure, but not in vast numbers like the other two. Also the French accepted the Native Americans, lived with them, and intermarried.
