Tell her that you are worried about her. Offer to go with her to a local clinic. In my town the clinic does all tests and physicals on a sliding scale, if you make little money, then you pay little fees or none. My father passed away from heart disease and my sister is the same way. She smokes and is overweight, but after speaking with her about how afraid I was that I was going to lose her too, I offered to go with her to the doctors office and now we make it a yearly sister day. We go to the doctor and then out to breakfast and shopping. If she thinks that you are concerned with what she needs to do and its in her best interest then she may well listen better and take better care of herself.
Hope the suggestion helps

Check into womens' clinics and Planned Parenthood in your area--she could get a pap done there without insurance and depending on her income it may be free. Just let her know that you love her and care for her and you know that if your mother were there she'd want her to take care of her health just like you do.

Unfortunately all you can do is insist she seeks help.

Does she have a job? Do they not have insurance coverage for her?

She could talk to a free clinic to get a checkup if she is on hard times (can't be too hard if she can afford smokes IMO).

Research and print out facts for her to look at, have her read them.

That is all you really can do. When and if she does have troubles down below, it is her own doing since she didn't bother getting checked out.
