You're going to have to be more specific. The term "Middle Ages" covers roughly a 1000 year period from 600 CE to 1600 CE. Also, where in the world geographically? Many trading routes existed, but there were some places where you could not get anything from Asia.
My personal list:
1. a first printing of Caucer's "Canterbury Tales"
2. a really good mead recipe
3. Egyptian anquities
4. an ivory Ma Jong set
5. a Norse Longship

Intriguing question. I think a hair dryer would be out. Let's see - - - - - -
1. A cache of oral antibiotics are a VERY good idea :
Cipro, Bactrim, Keflex, Amoxicillin
2. Precious gems - lighter than gold or silver and easier to
conceal - Would need hard money to get around
3. A small .38 pistol and ammo for self defense
in emergencies only. It would have to be concealed.
4. Inconspicuous clothing, boots, and backpack -
very well made and roughly in style with time and place.
5. A good supply of morphine tablets -small, easy to conceal,
and useful in a wide variety of medical situations if used
judiciously to help people.

1. Various types of anti-biotic and painkillers such as Penicillin and Morphine.

2. Cotton gin and a bag of cotton seeds.

3. Recipe for pizza.

4. Light Bulb, Laptop Computer and a Digital Camera

5. Soap and Shampoo.

What the Heck // anytime between 1000AD and 1500 AD I would most definitely bring a large bottle of Penicillin, A recipe for gun power plus simple instructions for making a gun, a hundred pounds of pepper, a hundred pounds of gold inside a stout chest on wheels, and a twenty five volume encylopedia, then I would set myself up as a Physician/Herbalist and live happily ever after.
