As far as the Merriam-Websters online dictionary goes a hookup means :

I have also heard it used as a way of expressing a wish to meet someone ("let's hook up at McDonalds") or to suggest that you have been with someone ( "Jess and I hooked up on the weekend").

hooking up and being together are totally different things... hooking up is like having a "boo"... you guys probably act and do everything like a couple but are not actually together

it really depends on how old you are. like, junior high its more like, oh we are officially going out, high school, its more like, went to a party and made out and what not, and college age is sex.
