The morgue. The local laws demand it. I will be without consciousness or awareness, so the destination is of little concern. Depending on where the organ donation program sends me, at least part of my physical organs will go to a few people and where ever that leads them.
Like a candle, the life/light will burn. Eventually it will stop.

That really depends on which religion you believe in and which version of that religion you believe in. I for one believe in reincarnation, which incidentally was once part of catholic dogma, (pre-middle ages).

Physically: either in a box and underneath some dirt or in a furnace to be made into ashes
Spiritually: Dunno, but you'll find out when you get there. Hopefully not soon
