They take their first breath when they cry. It actually closes a the valve between the heart and the lungs to allow O2 exchange. Thats why you want a strong cry. When the baby is in the womb it gets O2 through the umbilical cord via O2 enriched blood.

A baby will take his first breath when the outside air touches the baby's face. Many people believe that it is the passing through the birth canal that triggers a baby's first breath, but this is not the case. Look at water births...a baby born in the water and remaining under for a few minutes, will not take a breath. It is not until the baby is brought to the surface and his face comes in contact with the air, that she takes her first breath.
I gave birth to my fourth child in the water and he was under for several minutes, as soon as his face emerged he let out a CRY!!
There are also speculations that it is actually the umbilical cord being exposed to air that triggers a baby's first breath. This would make sense since it is the umbilical cord that has supplied all of the oxygen to the baby in the womb. Unfortunately, I was not able to locate, on the web, where I read about this theory.

They only breathe in the womb if their delivery takes too long or they are past their actual due date. They may take their first breath then. The trouble w/ that is that they may ingest some of their merconium and it will get into their lungs. The first breath is usually outside the womb when the doctor suctions their nose.

They practice breathing and crying in the womb. At birth, certain mechanisms change over as the lungs inflate. If the baby does not breathe on its own to at least partially inflate the lungs, there is nothing docs can do, because that mechanism won't switch over. (Discovery Channel).

Babies practice breathing in the womb but they arent really breathing. They are just exercising their lungs. When the baby is born it will take its first official breath and you will hear it cry. It is the most beautiful sound you can hear after labor and delivery. (It wont be so beautiful the 600th time you hear it though so savor that moment.)

There is no air in the womb, so obviously they are not breathing air. Until their face hits the air, they rely on the oxygen circulated in the mothers blood through the umbilical cord.

first breath is considered when they actually get a lung full of air at birth.
while in utero, they do gulp at the amniotic fluid, sort of like practice breathing and swallowing... but not considered a breath until air fills the lungs a second after birth.
