Your question is confusing. You can't just "appear" before a judge. There has to be something, like a case or a motion or hearing in the case, which is scheduled at a time and day by one of the parties or the party's lawyer, or, rarely, by the court.

You don't need a motion to represent yourself, if that's what you want to do.

And a letter is not used, except as a cover letter to tell the clerk what you are filing and when you would like it scheduled (some courts do their own scheduling, some leave it up to the lawyers).

If there is a local law library (in the county courthouse, usually) or local and state rules of court in the public library, you may be able to find a form for a motion for the particular court you want to be in or are in.

If you are appearing on your own behalf -- called "pro se" or "in pro per" -- you don't need to file a motion. You just need to appear and inform the judge that you are representing yourself.

If the court requires some written notice in the form of a letter, then just address it to the court, and make the short statement that you have considered your options and have decided to represent yourself.

In the legal system, the biggest mistake people make is to represent themselves. Immediately seek the advice of an attorney licensed in your state.
