Slope-intercept form is
Y = mX + b

So just rearrange your equation so that it is in that form. Move the Y term to the left. Move everything else to the right. If there are any factors/exponents, etc on the Y side, do whatever operations you need to do so that you are left with only a Y on the left. Whatever remains in front of the X is your slope and whatever else remains is your Y intercept.
