It could be arthritic bone spurs forming on any of the joints of the midfoot. It also cound be a broken bone.

Not having seen it, my bet would be arthritis unless she remembers some kind of trauma. If she doesn't want to get an x-ray then she will just have to deal with the pain and hope it doesn't get worse.

Not just human feet. Technically speaking, all feet are meant for walking. Centuries of footwear and surrogate transportation later, human feet have lost the conditioning to travel barefooted effortlessly. If you were to move around on your bare feet as aboriginal tribesmen and some African long distance runners do, surely the skin of your sole could be toughened over time and your legs would gain the endurance to take the pounding. In which case, blisters and muscle aching could very well become a thing of the past for you. Long walks are great ways to become lost in thought. The addition exercise is also beneficial to your health. Everything to gain, nothing to lose.

May or may not be broken. Could just be dislocated.
Only an x-ray can tell. Tell her that if she is too chicken to get a perfectly safe x-ray then she needs to stop whining about it.
