I guess it would be to "restore virginity" - I had a hymenectomy many years ago but not hymenoplasty. It was done by my gyn.
I found this info:
"Most surgeons only perform hymenoplasty or hymen repair (hymen restoration surgery) at the request of someone who needs the surgery for ethnic, cultural, or religious reasons. Also, most surgeons will not perform hymenoplasty surgery in a woman who has given birth.
Depending on your geographic location and choice of surgeon the price of hymenoplasty may range between $2500 - $4500."

Some women get it because they were raped and want to be put back in place. Others have had children and find sex is no longer enjoyable. I'm sure there are more reasons such as accidental tearings by falling or some other injuries to that area. As far as your other questions..I don't know.

look it up on google, and try and find a doctor who specializes in it.
I'm sure alot of surgeons will perform it, but its probably best if you get a more experienced one.
