Watch out for scams, no legit agency ever charges anything or tries to sell you classes or anything.

deb is wrong TheaterDoc is right. They want to know if you are seeking representation for TV & Film, or for Commercials. Usually Commercial representation includes print and modeling, but not always, so you should specify this also.
