I was going to pick her up at her place, drive over to the Clinic and get some cold medication while she waits for me in the lobby. Then I would drop her off at her place and go to bed early.

Most girls would not go for that. If I liked a guy I would not mind going with him to his doctors appointment if we did something else before or after. It is funny but extremely rude to pick up a girl, go to the doctors, then drop her off home. This behavior shows that your date with her is an inconvenience and you are trying to turn her down nicely by giving her a horrible date. The girl will break up with you or not call you back if you do this. This is not hot if it is the first date. If you have been dating for a while, she is comfortable with you, and you have had amazing dates then it will not be an entirely horrible date.
Overall this is not a hot date for most girls. Go out with the girl on a night that you do not have to go to the clinic and get medication. Just out of curiosity what is the medication for?

Uh, I was glad you specified it was for cold meds and not the "free clinic" Why don't you just cancel until you feel better, you're already contemplating making it an early night and you apparently have a cold, so she would probably appreciate you not passing the sniffles on to her.

1st date yes, other than that no. girls really expect to do it after the first date. she might hook up with someone else in the lobby and then you will be sad and have to take anti-depressants.
