Yes. You must apply for a change of name. There are state and local laws governing it; but, generally, you must fill out a form, run an ad in the paper informing the public and appear in court. You should have no problem, providing you have not been convicted of a crime. The fee is nominal.

Are you in the UK? You can have your name changed or add new ones at 16+ and change your name as many times as you like. My mum did it for me when I was nine. This is done by Change of Name Deed. You can also do it through Deed Poll but it's more expensive and you need to be at least 18.
May I suggest typing 'Change of Name Deed UK' in a search engine (if you have the Internet) to find where to get more information. A .gov.uk website will be more reliable than most.
Also, for more advice, go to your nearest Citizen's Advice Bureau. It's free and their excellent at dealing with various legal and social issues. They are friendly, informal and an advisor will deal with your case personally so you get the best out of the system. You can drop in or make an appointment.

I believe you could just contact your local Social Security Office. That is were people that get married go to legally change their last name. I'm sure that would be the same place to legally add a middle name. Search for your local office, then call and/or go down there and ask what you would need to do.
Helpful link below.
Good luck to you!

It first depends on whether or not you are considered a minor in you state. If you are, it may require your parents' approval.
Whether with their approval, or becasue you are an adult, you can change you name to just about anbything you want it to be. You may not be old enough to remember how Prince (the music star) changed his name to a symbol, becasue the rights to the name "Prince" were owned by a record label.
In any case, you could change your name to "Porcelain Soup Toaster", if that's what you really wanted. Hmmm, sounds like a good band name.
You'll have to contact your Local Government in order to find out how to do it. You'll have to fill out forms, but it's probably not overly complicated.

In most jurisdictions this is possible.
Apply to your local (or chosen ) Births, Marriages and Deaths Registrar (or equivalent civic authority) often located at Court Houses. You may need to provide alternates in case your primary choice is denied.
Complications may include having to notify a number of official identity dependant agencies like credit cards/ banking/ finance institutions, electoral rolls, deed/ ownership registers, insurers et al.
Remember it's your identity you are changing so check these first rather than after.

Go to the courthouse, and go to the place where they change people's names. In a big red box there will be a packet of gummyworms. EAT THEM. Thennnnn you will see a golden light. Walk towards it. Tell the genie the name you want to have. Pull a rice crispie treat out of your pocket and kiss it three times, fast. Then hum your favorite song, and walk out the door like nothing happened.

I think it's a simple name change operation. Find out what the laws are in your state for changing your name (in your case changing your name would mean adding one) and apply for the name change. Since you're 17, you may need parental consent. Best of luck to you!

lol, i dunno, I don't think so, I mean, the middle name has to be made legal at the time of your birth and also needs to be written on any documents that have relation to you so that'd be kind of a pain don't you think. I'm 16 and i want a middle name too! ha, but it woyuld rly be a lot of work. Because though ppl might not think a middle name is that imp. it's still(for some reason) required for evrything.

You can get not only one, but as many middle names as you like...well you may have to wait until you are 18. Do you have any good ideas?? Something plain, or are you going to go with something unique?

Yes! Just go down to the court house, and fill out the application and pay the fee! You may need your parent's permission.
