...in the rarely seen 1985 slasher film shocker, "The Mutilator", the last surviving guy and gal, severely cut, bruised, bleeding, and most desperate to get away, attempt to speed off in their vehicle; the car, initially failing to start, is shaken, when the killer tries to get at the couple, by thrusting a medieval axe randomly through the rag top of the car. Finally, the car starts, and the killer is shaken from the roof of the car, down to the protruding back bumper of the car; acting quickly, the couple presses hard on the gas pedal, and the car is thrust backwards, toward a brick wall, while the killer holds on, desperately. The car slams into the brick wall, and the killer is graphically cut in half by the bumper...

...and if that wasn't enough, a passing police vehicle approaches, just after these proceedings; the officer gets out, checks to see if the kids are O.K., goes to the back of the car, and sees the killer's body, cut in half...still holding onto the battle axe. Looking over to the kids, to ask what happened, the officer is unexpectedly and shockingly cut down by the killer, whose upper body comes to life, at the last shocking instant, and using the battle axe for one last blow, cuts off one of the officer's legs...

...alright, so it isn't the most dynamic movie death by car; however, as I recall, the audience watching the film DID jump out of their seats, to retrieve their hearts, which had jumped out of their chests, during that shocking and final moment in the film...

You could always check out Crash (1996 version, not the 2004 one!). It's about a man who, trying to give his relationship with his wife more "spice", turns to an underground group of car crash fetishists. The whole movie centers around this strange obsession, and so there are many crashes throughout the film, and injuries the fetishists carry around like medals. I remember an interesting scene where the 'leader' (Elias Koteas) of the group takes his camera to an intentional, reinacted crash scene, and takes photos for his collection of the gore, muttering to himself at the driver "why couldn't you wait for me?"

The first one that came to my mind was Taking Lives with Angelina Jolie and Ethan Hawke. I think it was in the beginning when that guy pushed or kicked that other guy right in front of that speeding car. That was brutal.

I can think of two.

First, The Crow - T-Bird drives his Thunderbird off a pier with a cool explosion at the end.

Second - The Godfather, and if your name is Sonny, don't be stopping at any tollbooths. It might kill ya!

Mahogany

The photographer (played by Anthony Perkins) goes berserk & tries to scare Mahogany (played by Diana Ross) for never before seen photos & ends up killing himself & badly injuring her.
