The "official" answer will be out in a timely fashion, depending on how fast the government works at it. Until that report comes out, any attempt to answer this question is just a stab in the dark and any correct answer is merely a lucky guess. Once the government issues its report then individuals or groups can criticize the entities in power for lack of inspections or maintenance.

Many people with engineering knowledge think it fell because it was not properly inspected and maintained.

It could be that various governments have diverted the money that was expected to be spent on such things to other purposes.

Too much money for unnecessary wars and rich people. Not enough money for inspection and maintenance of highway infrastructure.
