The Hall makes the final decision, based upon where the player had his lasting, historical impact, although they will consider the player's opinion.

The choice of which team’s logo appears on a player’s plaque is the Museum's decision, though we always consider the wishes of an inductee. As a history Museum, it's important that the logo be emblematic of the historical accomplishments of that player's career. A player's election to the Hall of Fame is a career achievement, and as such, every team for whom he played is listed on the plaque; however, the logo selection is based on where that player makes his most indelible mark.

Some of the earlier answers are correct---the final decision rests with the Hall of Fame. But not all players have a particular team's hat on their head for the plaque. For example, Jim "Catfish" Hunter had many good years for both the A's and the Yankees. He could not make up his mind which hat to have on his head, so he does not have either logo on his plaque.
Wade Boggs and rumors of David Winfield deals brought the whole issue to light a few years ago. When Wade Boggs agreed to have a Devil Rays hat on for cash it was the last straw.

Well, the answer from teh Hall of Fame is right above mine.
I'll just add that they changed it because of Wade Boggs. After an all-star career in Boston and winning a WS ring with the Yankees he signed a contract to play for Tampa Bay and part of his contract said that if he got into the hall of fame, he'd wear a Devil Rays hat. Boggs said he wanted to do that because he grew up in Tampa but the Hall thought it was tawdry - that Tampa was paying him extra to get it's hat represented in the Hall. Then they changed the rule.

It used to be that the player decided, but when some started getting contracts with the stipulation that if elected to the HOF, they would have to represent their new team, that was eliminated. Now, the HOF has the ultimate say although they will likely still consider the input of the honoree.
Luckily, that didn't matter this past year because both inductees only played for one team.

players used to be able to pick until wade "kentucky fried" boggs signed a contract with tampa bay with a monetary bonus to wear the Drays cap to the hall. the hall said hell no. so the hall put him in with a bosox cap on. the hall will take the players preference into consideration, but with rocket, if he said the jays or the stros, the hall would probably say no. either the bosox or yanks, i am guessing he'll pick the yanks over the sox.

Excellent question. I don't know the answer. I would think the HOF, would somehow find a way, so they will show all the teams they played with. I do know it does take 10 years, before they are voted on.
