How would U.S. handle it, would we plunge into civil war, would we nuke back?

We would nuke back and probably the whole world would die.

This is one of those questions which is nearly impossible to answer in the abstract. Certainly if the entity causing the bomb drop were known, nuking them out of existence would be a real possibility.
Even if this were to occur it is highly unlikely that any other entity would react against the United States for two reasons.
1 - If this act went unpunished other countries know that they would likely be next.
2. - No other country has the number of atomic weapons with state of the art delivery systems as does the United States. What most don't seem to understand is that if the United States wanted to do so they could bring the entire world to heel over night. However, that is not what the United States wants to do and what it does it does with great restraint.
