In this situation where saying or doing something seems awkward at first, I would write a letter saying how happy I am for having back the relationship.Say how good being back together has been for you, what has changed in you after having each other back, what has happened since your separation.I would add an apology or a forgiviving "tone" to my letter just to repair what hurtings were said and done. Then end it with a hope that things would only get better from hereon .
There's nothing like sharing your immediate thoughts to start a more promising togetherness.

I suppose it depends on why you were separated and why you rejoined. If you thought you needed time apart and got back together, then I agree with the weekend trip idea. If one of you cheated or something like that, I would say it's a mistake to get back together at all because there would always be trust issues.

I would ask why the separation, what was the issue that caused the two of you to separate and then rejoin? Counseling as silly as it seems opens the door to open and honest communication, a vital skill needed for any relationship to survive.

Start things out slowly. Don't try to get to the desert menu without having the salad first.

By that I mean you need to let the other person know that what happened between you the first time was nobody's fault.

Keep everything friendly and open. You will know when it's time to move to the next level.

no person can REPAIR a relationship of any kind......it's the LOVE that makes the heart grow fonder. and if that LOVE is strong enough, together with time it will do the healing. these things no one can force. it's there or it isn't. if it's not there don't worried. just continue to live as if it never was..
best hopes to ya......

1st thing is to stop worrying that it may not work out. Start doing fun things that you both enjoyed doing when you met - going out, trips, dinners whatever. Soon you'll settle back in and it wont feel like such a big deal

Discuss the problems which caused the break-up in order to see if they can be dealt with. Don't pretend they didn't exist.

Then discuss the reasons why you want to get back together.

Try a mutual endeavor. Go to church together and listen and discuss God in your lives.
