All your questions will depend on what you're using to treat the white spot (ich). You can use a medication (something containing the ingredients malachite green and formalin will be the best), or use salt (you preferably want salt that doesn't contain iodine - there's an aquarium salt, but kosher salt, rock salt, canning salt, pickling salt, and non-iodized table salt are the same thing and cost less). For medications, the directions for each will tell you how much to add. If you have fish that are sensitive to salt or medications in your tank (tetras, catfish, or scaleless fish) you want to start with only half the recommended dose, and build up slowly. If the medication comes in a bottle with a dropper, you can add it a drop or two at a time where the water comes out of your filter. If it's a powder or tablet, mix it with some tank water in a separate container first (same for salt). Salt should be used at 1-3 teaspoons per gallon (depending on the fish sensitivity) - platys can tolerate a high level, as can most livebearers.

Remember to remove your carbon if treating with a medication (it's not necessary if you treat with salt).

It also helps to raise your water temperature to at least 86oF while treating (do this slowly by raising the temperature a few degrees each hour, and lower it the same way when treatment is over). This speeds up the life cycle, so the parasites drop off the fish faster so they can be killed by the medication/treatment in the water.

Your fish has a very common disease called ick or ich. It's very easily treatable if cought early. Go to your local aquarium shop and ask for an ick treatment such as "Quick Cure". Follow the directions on the label according to tank size and number of fish.
