B is in the active voice. How can you tell? If the verb "to be" is in the sentence, it's in the passive voice. Forms of the verb include: "be, am, is, are.

A, C, and D are all in passive voice. This is evidenced by having a form of the verb "to be" plus another verb in the past participle; "are witnessed," "are considered," and both "is said" and "can be taken" in the last sentence. As a reality check, try the verbs with the subjects: the accidents aren't doing the witnessing; the accidents aren't doing the considering, "it" isn't doing the saying, and the steps aren't doing the taking--passive voice.

I think it's B. "However, few people think seriously of doing something about them." It's active because the subject of the sentence ("few people") is the actor of the verb ("think"). I suppose it could technically be A, also, but C and D both have passive verb phrases. So B is the best answer. But it's been a long time since I've had to do this...

B. is the only active phrase, but it is not a stand alone sentence because it begins with a conjunction. A. and C. are complete sentences, but they are both in the passive voice. D. is a conjuctive phrase, but it too is passive.
