Once you've unlocked a cell phone, is it unlocked forever? Or do you have to unlock it every time a new SIM card is used with it?

Once it is unlocked it will remain unlocked. Does not matter if you change SIM cards.

Locking and unlocking a cell phone is a personal option. It is for security. You would lock the phone so that someone else could not use it without the code that you set. When you buy a new phone, you may have to "unlock" it by using a provided code. The same with a new sim card. It must be activated.
